It sure looked like it to me. Either that, or it wasn't shot very well. I mean, given how apologetic the other guy was, Keith must have been moving with the right of way, but it sure looked like he was stopped. Maybe he was waiting to make a left turn.
I'm not sure what was more disappointing for me, that he would decide to have an affair with a married woman, or that it took a near death experience to make him do it.
1 - Waiting to make a left turn was my first thought. But there was no oncoming car for Keith to wait for, nor did one arrive after the crash, and he didn't have his turn signal on. It just seemed like Keith was parked in the intersection.
2 - Cancellation rumors, like Hot Stove rumors, are merely rumors until fact. :)
1, 3 - I was riding the exercize bike at the time I was watching it, but my impression was that he was supposed to be moving through the intersection, but in order to get the kind of collision they wanted, they had to film it with a parked car with nobody in it. In other words, poorly shot. Other than that, a pretty good episode.
4 - Have you not noticed the size of Laura San Giacomo's, er, shirt? I'm not real into huge, uh, shirts on women who otherwise look as if they long ago passed the last rest stop on a four-day drive through the Yukon, but some people are. So there's that.
I NEVER liked her (basically only knew her from whatever that david spade show was)...and always wondered the same thing as 4, until I finally saw S,L&V a few months back. She was very good in that. Still not a fan of her now, but I'll grant you 8, definitely.

But apparently the Enrico Colantoni-Laura San Giacomo pairing are the Tracy and Hepburn of the 21st Century.
